Specialist Mental Health Support
OUR SERVICES
Specialist mental health support at home
Registered Mental Health Nurses (RMNs) support hospital discharge and crisis care at home. We work with NHS teams, ICBs, local authorities and families to arrange clinically assessed packages, including support that reflects learning disabilities or autism.
Tell us which RMN competencies the person needs, including risk assessment, medication support, de-escalation and any specialist training. Our clinical team reviews the referral and agrees suitable staffing and a safe care plan with the professionals involved.
For commissioned referrals, include the agreed funding route, such as Section 117 aftercare or NHS Continuing Healthcare, and the responsible commissioning contact. Suitability and funding must be confirmed for each package.
For urgent referrals, call 0203 225 5579. We confirm availability and a realistic start date after reviewing the referral; submitting an enquiry does not reserve staff. Email referrals@homecare-providers.co.uk with the location, required start date and a brief overview.
Tailored Mental Health Care
1. Discuss the referral
Tell us about the person’s needs, current setting, location and proposed start date. We can also assess urgent gaps where an existing care package is at risk.
2. Agree a safe care plan
Our clinical team reviews the referral with the professionals involved and agrees staffing, risk management and a realistic start date.
3. Put support in place
Qualified mental health nurses provide care around the person’s routines, preferences and goals, with clinical oversight and regular review. Support can also reflect learning disabilities or autism.
