We provide nurse led care at home for children and young people with complex health needs. Support is built around the child, their family and their daily routines, with a care plan agreed following assessment.
Care may include respiratory support, tracheostomy and ventilation care, medication support, catheter and stoma care, and support for learning disabilities. Nurses and care staff are matched to the clinical skills and competencies needed for each package.
We work alongside the child’s existing clinical team and follow their agreed treatment and medication plans. We can also help families plan discharge home, respite and the transition to adult services.
